Some members of Dental Health Services, a California Company, and Dental Health Services, Inc., a Washington Company may have had personal information disclosed in a privacy incident.
What Happened?
On October 22, 2024, we discovered that some invoices were mistakenly printed double-sided. As a result, certain members may have received another member's invoice printed on the reverse side of their own.
What Information Was Involved?
The information that may have been inadvertently disclosed includes your name, address, Plan ID number, and member ID number. No Social Security numbers, birthdates, or clinical information was involved in this incident.
What Are We Doing?
Upon discovering this issue, we immediately took steps to prevent further incidents of this nature. We have implemented additional checks in our printing processes to avoid double-sided printing for sensitive documents. Additionally, we are reviewing our procedures to enhance the security and privacy of member information.
What You Can Do?
While we believe the risk of misuse is low, we recommend that you remain vigilant in monitoring your Dental Health Services account for any unauthorized activity. If you received November's invoice, please destroy the double-sided invoice immediately. If you are just receiving your November invoice today, your information as described in the announcement above went to another member.
